"Monitoring the health of women of childbearing age at the level of Primary Health Care"

October 29, Karaganda State Medical University and the Health Department of the Karaganda region with the participation of the vice-rector for clinical work and CPD B.N. Kosherova, Deputy Head of Children’s and Maternity G.K. Mamalinova, the teachers aware of the university held a workshop for deputy heads of medical organizations, obstetrician-gynecologists, general practitioners, therapists on the theme: “Monitoring the health of women of childbearing age at the Primary Health Care level”.   Issues on the conduct of women of childbearing age at the level of primary health care, family planning and also early detection of extragenital pathology and for improving of women health with chronic non-communicable diseases at an outpatient level was discussed on the workshop.    Professor L.I. Arystan provided with existing legal and regulatory instruments and standards for the provision of medical care to women of childbearing age in the Republic of Kazakhstan. She presented the results of her own research on the structure of  Extragenital Pathology (EP) in pregnant women, dynamic observation of general medical care (GMC), interprofessional interaction and teamwork.        

Professor D.ZH. Tayzhanova - Head of the Department of Medical Practice No. 1, an independent expert of the Ministry of Healthcare and Social Development, elaborated in detail on the results of a system analysis of clinical cases of maternal mortality in the Republic of Kazakhstan. She presented in detail the clinical cases of pregnant women with the most common  Extragenital Diseases (PE, sepsis, SARS, etc.). Risk factors that aggravate the course of pregnancy are arterial hypertension, anemia, obesity, anemia, subclinical hypothyroidism, chronic kidney disease. To provide quality medical care, Professor D.Zh. Tayzhanova, noted that doctors should collect a detailed history of life, conduct a differential diagnosis with a wide range of diseases, strictly pregnant women should conduct in accordance with clinical protocols of diagnosis and treatment, pay attention to interprofessional cooperation to improve the quality of medical services.   I.L. Kopobayeva is an assistant of the Department of Obstetrics and Gynecology, she presented the indicators of maternal mortality rate in the Republic by regions and single-industry towns of the region. She noted that it is necessary to improve the dynamic observation of pregnant women in primary health care, medical documentation, as well as in the area of improved material and technical equipment of the organization of obstetric aid. Thus, in 2016-2017, medical equipment was purchased in the amount of 249.2 million tenge. In order to eliminate the staffing shortages provided funds for training at the residency. Housing for young specialists was repaired and provided, as well as the “Virtual Headquarters for Prevention of Maternal Mortality” operates. The mobile applications of Damu Med is a “Module for Pregnant Women”, a high-risk office is functioning in the city of Zhezkazgan.   Professor M.M. Tusupbekova presented a clinical and morphological assessment of the quality of diagnostics in the pathology of pregnant women on the example of clinical cases (pulmonary embolism, sepsis, tuberculosis, iatrogeny, etc.). She gave a very detailed review on the structuring of the diagnosis.   

Vice-Rector B.N. Kosherova pointed out in her speech that a joint master plan for the implementation of an integrated model of the service of obstetrics and childhood was developed in Karaganda region with the Health Administration of the Karaganda region, curators of districts and mono-cities were fixed; professors and teachers of the university participate in the consilium of complex and difficult cases in pregnant women, women in labor, postpartum women and children at the request of the Health Administration of Karaganda region and medical organizations. Professors and teachers of the university are members of the regional headquarters in taking urgent measures to reduce infant and prevent maternal mortality. The university clinic and dental clinic provides treatment-diagnostic and advisory assistance to women of childbearing age, pregnant women and children in the framework of the guaranteed package of free medical care (GFMP). Currently, for the provision of medical care there are 27 residents in the medical organizations of the region and the republic.    B.N. Kosherova noted that the university conducts advanced training courses: Extragenital Pathology (EP), family planning and contraception, examination algorithms and pregravid preparation of women of childbearing age, prevention, diagnosis and treatment of obstetric pathology and gynecological diseases in the primary health care system, endocrine gynecology, including pediatric, antenatal care, cardiotogram of diagnosing the threatened state of the fetus during pregnancy and childbirth, emergency conditions in obstetrics (obstetric ovotecheniya, pre-eclampsia, eclampsia, sepsis).  Deputy Head of the region G.K. Mamalinova noted that the existing problematic issues are resolved, much work had been done by the health department for training doctors and IDA, for the purchase of modern equipment, thanked the leadership and faculty of the university for their joint work.
